PIEAS runs a 4-point scale topping out at A+. Published grade points, unpublished marks table. Islamabad, 1967.
There are 10 grade tiers. Below D the course earns you nothing.
No conversion formula for PIEAS appears in any source we can read. An equivalence only counts if the examination office issued it.
BS rules explicitly use a 4-point scale; subject, semester and degree passing thresholds are separately defined. We would rather leave the gap than fill it with another institution's figure.
The usual culprit for a mismatch is a departmental exception. Where yours disagrees, go with the registrar. Forward the document and the page gets fixed.

BS rules explicitly use a 4-point scale; subject, semester and degree passing thresholds are separately defined. Percentage bands come from Table 2 of the PIEAS BS academic rules. A mark of 45 to 49 earns GP 1.0 and counts toward the GPA, but the course must still be repeated to reach the C grade needed to pass.

The scale is in the table above, taken directly from the PIEAS source we cite. The linked source is where each grade point in that table came from.
Not unless the relevant university or receiving authority explicitly publishes that formula for the applicable purpose. If your department publishes something different, that version governs your transcript.
