UoM uses 10 grade tiers on a 4.2-point scale. It has operated in Moratuwa since 1972.
There are 10 grade tiers. Below D the course earns you nothing.
There is no published percentage mapping. Ask the examination office before you write a figure on a form that carries a signature.
The usual culprit for a mismatch is a departmental exception. Where yours disagrees, go with the registrar. A published grade point with no published band behind it is only half a policy. We will update this page as soon as someone sends the document.

Credit-weighted, as at most institutions. Each grade point is multiplied by the course's credit hours, the results are summed, and the total is divided by credits attempted. A naive average of grade points gives the wrong answer at UoM whenever course credits differ.
Not from the grading table alone. Gradometer keeps grade assignment separate from reverse CGPA-to-percentage conversion unless an official formula is verified. The linked source is where each grade point in that table came from.
